As some of you already know, I have accepted a position in North Carolina and will be closing my practice on October 31, 2009. Dr. Edward Leaton and Dr. Richard Waller of Central Virginia Neurology have kindly agreed to provide follow-up for my patients and to transfer records after I leave for those of you who choose to follow up with a neurologist outside of their practice. Below are instructions and contact information regarding this transition. If you have an active neurologic problem and anticipate the need to communicate with a physician in November after I leave, please arrange for an appointment with your new neurologist as soon as possible.

Please be aware that any request for transfer of records must be accompanied by a release signed by the patient or legal guardian. A copy of the request form can be found by clickingon the “Medical Release Forms” link to the left. You may fax or mail the release to the appropriate party. Those of you who choose to follow up with Dr. Leaton or Dr. Waller will not need to fill out a Medical Record Release form.
